For some reason, I am having extra (blank pages show up.) The index page only contains a few entries and I have placed it at the end, and there is an entire blank page. Also, the same occurs before the TOC page (a single blank page at the beginning of the project). I have checked the chapter settings, all seems ok. Any suggestions?

In your TOC for your PDF output, open the properties for one of the items that is being followed by the blank page. I assume you've got an "Index" book or page on your TOC. Right-click it to open its properties. (These are its TOC properties, not the index topic's properties.) Switch to the Printed Output tab. Under Chapter Break, I assume that you've set the Index to start a new chapter.
How have you got the "Auto-end of 'left' page" setting set? If it's "Enabled," that's what's giving you the blank page. Disable that setting. When you regenerate your PDF, that blank page following the index should be gone.
Do the same for the other items on your TOC after which you want to disable ending on a left page.
