Duplicitous TOC

Duplicitous TOC

For some reason, on a PDF output, I am getting a duplicitous Table of Contents page. One is kind of "ghosted" and is a blank page in the view pane of the PDF reader. The other is a populated TOC with contents in the view pane of the PDF reader. I have looked at the TOC and the Target, but cannot seem to find what is causing this. Any suggestions or is anyone else having the same issue?

does it look like anything is out of place in your toc topic (where the proxy is)? an extra line?
