I noted that, in your own Help files, the topic name appears to be embedded in the breadcrumbs proxy, rather than in the body of the text. I like the effect because it locks the topic into place (rather than having it scroll out of sight).
I haven't been able to figure out how that was accomplished. Perhaps it's an illusion -- the breadcrumbs proxy is formatted so that it appears to blend seamlessly into the heading, and the heading itself has been set not to scroll? That's just a guess. How was it accomplished?

The "About the Breadcrumbs Proxy" is the heading in the topic, an <h1> tag. To get the effect you're seeing, they set the background color of the breadcrumb proxy to be the same as the background color for the h1 tag. They also removed the bottom border of the breadcrumb proxy and set the top, left, and right margins of the breadcrumb proxy and the h1 tag to 0 so the background color would go all the way to the edges of the page (they use padding to position the text away from the page edge).
So there are two separate paragraphs/DIVs but their styles were adjusted in such a way to make it appear as though they're in one DIV.
There might be a better link for downloading MadCap's stylesheet for their help, but you can get to it by going to http://webhelp.madcapsoftware.com/flare ... styles.css, if you want to take a look at their styles, at least for topics.
