Compiler (Internal Error): The node to be inserted is from a

I got this error

Compiler (Internal Error): The node to be inserted is from a different document context.

while trying to build a Word version of my project. I can output to PDF OK.
Its a bit of a noodle-scratcher as I've never come across it before.
Any hints on how isolate the cause?

I could speculate as to what that means, but I have no idea how to go about troubleshooting it (aside from the generic method of removing topics one by one until it works, which would tell you which topic(s) has the problem).

I'd contact MadCap support if you have a contract, or report it as a bug if you don't.

Update:
Looking at the log, I progressively deleted content from the topic where the thing failed until there was only the html header gubbins was left. It still produced the error message.
I then removed the topic from the ToC. The error then appeared on the next topic.
I then removed that topic from the ToC. The error still appears.
Thanks for your advice, Andrew. I'm now going to put in a support request and will post the solution when available.
