I have inserted xrefs to autonumbered table and image captions, using:
1. insert bookmark at end of target para
2. insert xref with mc-format {paranumonly} linked to the relevant bookmark
In some cases they are showing in the output (webhelp & pdf), some cases not (in either output).
This one does not appear in output:

<MadCap:xref href="#Power and reset Pins" target="" title="" alt="" class="caption">Table 1</MadCap:xref> shows power, ground and reset pins available on the module connector.</p>
<p class="caption_table" MadCap:autonum="Table 1 - ">Power and Reset Pins<a name="Power and reset Pins"></a></p>
<table style= blah etc.
This one does:

<MadCap:xref href="#pinout" target="" title="" alt="" class="caption">Table 1</MadCap:xref> below shows the signals present on the Brooklyn II connector.</p>

<table style= blah etc.
As far as I can see, they're the same. However, I am slightly hungover so I may have missed something.
Anyone got any ideas? a bug maybe?

note - it shows fine in the editor (Table 1 shows power, ground and reset pins available on the module connector.)

Did you insert the bookmark manually or let Flare do it automatically when you inserted the xref? Considering that you're xref-ing to a paragraph and not a heading, I'm betting it was the former, in which case the problem is that your bookmark has spaces in it and that doesn't work. The Insert Bookmark screen has a note at the top not to use spaces in the bookmark name, but it doesn't prevent you from adding them (why, I don't know, since that would be a pretty simple key block in that field). Anyway, the second one works because your bookmark is one word. The first one doesn't work because of the spaces in the bookmark name. Remove those spaces in both the bookmark and the xref link and it should work.

Yep that's fixed it, thanks very much.
I was doing it manually, and I didn't see the 'do not use spaces' note (probably due to aforementioned hangover) but yes I think it's slightly bizarre that Flare haven't tweaked that dialog to reject spaces.
But I'm up & running now anyway.
