Different TOC results for file access and URL access

I have an HTML5 website that I am seeing conflicting results. Even though I update the project and the output looks great, when I publish the output to my web server, the TOC does not display correctly; it is missing some new links. When I click on the default.htm file from the web server directory, it looks good, but when accessed from a URL it is not updated.

I also deleted all the contents in the web server documentation directory and re-published. Same results....

Is there some sort of web setting that is causing this?

Just in general the first thing I would look at is clearing the browser cache. (It's something I always forget, so I finally stuck a Post-It to the top of my monitor to remind me. It works [the Post-It] about 50% of the time.)

I hope that's all it is!
