Why does the ruler in image 1 look different from the ruler in image 2?
Will this harm anything?

Different in what way, magnification? Otherwise, they look the same to me.
By how the text looks in the two images, I'm guessing that the two topics are being displayed in the XML Editor at different magnifications. You can check (and change) the magnification with a control at the bottom of the XML Editor.

They look different in that the body of text, and the horizontal ruler, appear moved over to the right. Not sure why as yet.

It's based on the page margins, which is based on your body tag and/or the medium. If the print target is your default target and your page layout has the margin starting a .5in, for example, then you'll see the gap between the page edge and the margin edge. If the default target is an online target, then you might have a difference in gap if you're using different masterpages that point to different body classes, for example, although that one probably isn't as likely.

I think it leaves room to show the structure bars (if they are turned on).
In your first example, do you have some text further down that we can't see with several levels of bullets, or a table, which uses lots of "structure bar" space?

SOMETIMES I can fix that by switching the medium to and fro, but normally I just live with it. - And of course I reported a bug ... some 2 or 3 years ago ... maybe you should do that too: https://www.madcapsoftware.com/feedback/bugs.aspx
