Image is excluded?

Can anyone tell me why the image in this code is being excluded? I'm running Flare 8.1.2. The text before it and the table behind it are printing, but not the image.

There are no conditions in this file. I published the doc a month ago and the image was there. I'm sure this is going to be a simple thing that I'm just not seeing. Any ideas?

Two things to check
- does the image file still exist in that location
- did anything change in the class MaxWidth5Inches

And:
- Has a folder in that path, or the png itself, got a condition that is excluded in the target?

Thank you both for replying. I looked at all three options and it turned out that the graphic file itself had an incorrect condition applied. Thanks for the 'extra eyes."
