The footers are large enough for the content.
There does not appear to be any body or decoration frames in the way.
What might be some other reasons why the footer text (manual title and page number) is included in some but not all of the PDFs?

Do you have that footer set in every page of the page layout you're using? Could some of the topics be pointing to the wrong page or page layout? For example, I don't have a header frame on my First Page and Title pages of my page layouts, so if I set a chapter or page layout break on a TOC entry and point to one of those pages, then I wouldn't get a header on that topic in the output. Perhaps something like that is happening to you.
Also, do you have multiple page layouts? If so, did you specify one of them as the default page layout in the target and that page layout doesn't have footers on every page?
Lastly, do you have the option to auto-end on left page enabled in your TOC entries? If so, do you have an Empty page defined in your page layout? I'm not sure what happens if you have the auto-end on left option enabled but don't have an Empty page in the page layout. Flare could not use a blank page instead or could refer to the Normal or Left/Right page. Or it could simulate what the effect would be if you used the default Empty page design, which I think is normally blank except for a body frame. So if you have that option enabled but don't have an Empty page in your page layout, then add an Empty page type and add the footer to it.

A quick update:
We have several issues going on; however, I believe the main source of our problem is page breaks in <h1> and the tables. I removed style=page-break-before: always;" variables from the <h1> tag, and this fixed it until the topic hit a table, which does not want to break correctly across the pages. This has to be a bug, right? Why would setting page breaks in tables so that they tables break correctly in the end product interfere with footer content?
Also, the problem is occurring in the middle of a topic. The topic in question contains three tables, and one of the tables breaks across two pages. The default is the break used in the table CSS. From that point forward, no footer content. Previous pages contain the footer, especially after removing the page breaks from the <h1> tag.

What version of Flare are you using? I seem to recall some problems in earlier versions of Flare where the table extended too far into the footer area and was covering it up. I wonder if that's what's happening to you. I thought that problem had been resolved, but perhaps not.
Are you able to send your project to MadCap Support? They might be better able look at your stylesheet, page layouts, and topics to see where the problem is, or if it is indeed a bug. I know I don't have a problem with my footers being covered up by tables that split across pages and I have page-break-before:always set on my h1 tags. I can't remember offhand if I controlled where the tables break or if Flare was doing that.

We're on V.9.1.2 after recently upgrading from V8.

I haven't yet contacted support, but that is on the list if all else fails.
Checking the breaks in the tables is also on the list, but I don't think that's it. Why? The problem occurs with a specific topic file, which may or may not be shared among several guides. The problem occurs in some of the targets but not in all of them. What this means is that when the topic file is included in some docs, the target generates just fine with footer content. Other docs that include this topic file do not have footer content from that point forward.
I've toyed with the TOCs ad nauseum, trying every known setup I can, ensuring the right page layouts are linked, etc.
I didn't set any of the doc projects up; they were converted from Framemaker by my predecessor. What tends to happen is when I make changes regarding how the files are organized or to the TOC, issues like this can pop up. I expect these types of things now and then until I'm able to optimize it the way I want to manage it.
I'll check the breaks in the tables; although I don't think that's it. The avoid break inside rings a bell; I've seen that before but can't recall where off the top of my head.

Also, the footer content can go missing mid-topic onward. I've looked at the code and found nothing out of the ordinary. MadCap.com is down. I'll contact support tomorrow.

Thanks! What is completely mindboggling is that the footer content stops showing up mid-topic in some cases. It makes NO sense whatsoever.
