Was looking at the topic via the text editor and decided to check out the xsd file that is referred there:
http://www.madcapsoftware.com/Schemas/MadCap.xsd
The result was an Oops, Error 404 page on the MadCap Web site.
If it isn't accessible, what are the implications? What actually is this file used for? Is it common practice to customize this/an XML schema document?

This isn't a file that can be viewed in a browser. It's a schema definition file and defines some of the MadCap-specific tags and structures used in the Flare XHTML. You can't view a CSS file directly in your browser either (I don't think), simply because that's not how it is designed to be used.
Your project will be able to find it when it asks for the information in it in the way it is meant to be accessed, whatever that is. I can't give you much more detail, without doing a bit more research myself, perhaps others here can, but fear not, all is well.

I'm not sure if it actually goes out and looks for the file. If MadCap is using MadCap.xsd, then I should be able to add element tags to it, as I would expect from an XML editor. After having dug out the local copy of MadCap.xsd and adding some stuff, I did not find those elements supported in the editor, Is the editor actually using the file or is it hard-coded and only there for reference purposes?
