Vertical Chapter Titles

So, I love the way MadCap has their Chapter Titles and TOC (see attachment). Does anyone know how I would go about mimicing that look -- I mean the Chapter Number and the title specifically? I thought if I tried to design it in Fireworks and bring the CSS over - but Fireworks only likes to export HTML.

That template is also supplied with Flare in the Factory Templates; e.g. Online and Print Advanced, and User Guide Advanced.
