Need to find and remove "Ghost Phrase"

I have phrases that are listed in the index, but that aren't in the text. I have opened up the pages in the text editor, but I still can't find the "ghost phrases." Is there a way I can see and edit a list of all the words and phrases that are in the index?

Clicking View > Index Window should Display all the index entries in a project
