This will sound weird, but:
I have a project with several "manuals" folders. The manuals all share the same style sheets, master pages, page layouts, common images, etc. There is a master TOC, but the child TOCs all have the same properties. However, one manual set is not showing the breadcrumbs or context menu that appear in the other manuals when compiled. How can this be, when all the docs have the same associations and share the same master page? The menu proxy and breadcrumbs proxy are not conditionalized. This is a really simple project. What am I missing here? (My only defense is that I've been up all night.)

I had something similar happen recently, and if memory serves it was a TOC setting (or rather, lack of a TOC setting). On the master TOC, check the TOC properties for the child TOC for the output that isn't building the breadcrumb. Check the Advanced tab and make sure that you've selected the check box about replacing. (Sorry, I'm not on Flare at the moment...) You might also need to select Replace from the drop-down list (though I'm not certain about that).

Flare won't generate breadcrumbs or the menu proxy if they're empty.
So if they're not appearing in topics, check that the topics are included in a TOC, and that the TOC is being included in your target.
As Nita suggests, check the properties of the link to the child TOC (in your master TOC) - you probably don't want anything set on the Auto-generate tab.

I'd also look at the topic that is associated with the first TOC item on the TOC of the breadcrumb-less child. Perhaps something has happened to that topic so that it is no longer valid XML.
