Putting Checkboxes into a Flare Doc intended for PDF output

I'm trying to add checkboxes to a topic so that employees performing scheduled maintenance can "check off" tasks as they are complete. They can then either email the pdf or print out the checklist and send it to management. I can't get the check boxes to display either in Flare or the build. Here is the code I put into the text editor:

And the pdf file only displays the text. Am I doing something wrong?

I was sure I had inserted checkboxes into one of my topics and I have. I think I just selected Insert character and then selected a checkbox Wingding. It displays fine both in Flare and in the HTML5 output.

That's a HTML form, so it'll work in a browser, but not in a PDF.

I know you can create PDFs that can be filled-in, but I don't know if you can produce them directly from Flare, or whether you need to use Acrobat too. Maybe someone who knows better might chip in.

I have check boxes in some topics using an inserted symbol. Here's my code:

It works in both HTML and PDF. You can make the PDF output from Flare fillable in Adobe if you need to.
