help comment add-in

What add-ins are you all using to allow comments from the public on your help outputs?

I was looking at the Flare showcase (https://docs.openx.com/Content/publishers/bidder.html) - the OpenX help allows you to comment without logging in. Screenshot below.

MadCap Pulse requires a user profile if you want to comment on the help, so we need to look elsewhere.

It looks like they use wufoo forms to display the form and collect the data.
