form-linked button pulling data from HTML page title?

Hi folks - I just got an interesting request for a mnior addition to the help sys I'm building here, and it sounds wonderfully logical and simple in concept, but I'm not sure how I'd approach it within Flare, or if this represents something best done outside Flare post-publishing... so I thought I'd see what the community here thought about this idea.

This idea hinges on the fact that on each topic page, in the side menu, there will be a big, friendly button with "Don't Panic!" on it... sorry, with "Support" on it, which is linked to an embedded SmartSheets form page, so that at any point the user can easily jump to putting in a support ticket. Question is, can we easily sample the topic title (HTML 5 page title, or H1 title) of the page on which the button is smacked such that it pre-populates a "topic" field in the embedded SmartSheets form?

I posted this same query on the Slack #madcapflareusergroups thread too for good measure.

The challenge is that we're hoping for a somewhat plain-vanilla out-of-the-box solution so that it's easily documented and supported, rather than a custom scripting approach - but I've no idea how reasonable a goal that is given the specificity of the idea!

It's easy to get that information - e.g.
document.title - will give you the topic title, which by default Flare sets to match the first heading
document.location.href - will give you the topic URL

So you just need to find out how to populate the form fields with this info.
Is this sort of example not available on smartsheets site/forum?
