What do others put on a cover page of a Madcap Flare pdf?

That's a VERY broad question.

I put on cover pages for my Flare-generated PDF output whatever is required by the client. The content might include, just to name few things:
-- Publication title
-- Last generation date
-- Publication number
-- Publication revision number
-- Publication revision date
-- Prepared by
-- Approved by
-- Prepared for
-- Company name
-- Company logo
-- Artwork

Similar to what Nita said, I typically include our company logo, title, and usually the date for a subtitle. Sometimes we'll include a photo on the cover along with the other items I mentioned. For consistency, most of our cover pages for different documents are the same besides swapping out the title of the document and the date, or sometimes a different photo, but the placement and branding/colors, fonts, etc. are all the same.

Depends what sort of document it is.

I do customer-facing PDFs, whose front pages (covers, if they were being printed) include:

Company logo and tagline
Product title
Release name
Release date (on some docs)

Other info that appears on the second page includes:

Copyright date
Copyright text
Contact information
Doc number
Build number and date
