heading 2 on page does not appear in TOC of output

I have a h1 topic with h2s on the same page - but they don't appear in the TOC??

I'm pretty sure this has worked in the past, or am I imagining it? Either forgot where this setting is, broke something, or Madcap changed something, help appreciated.

Could it be a class of h2 rather than just a bog-standard one?
