Does anyone know occasionally when our topics are edited, a <h1> tag will change to <h1 MadCap:autonum="1 ">? Is this set as a per-use setting?

What is in your stylesheet for h1? If your stylesheet applies an autonum to a h1, then this is absolutely fine in your topic, because it's what you eventually want in your output.

Yup, it's your Print medium that's doing it.
I agree, this is not ideal behaviour from Flare and is just another way to add confusion (a bit like when you change from one medium to another and then insert a x-ref) but it won't do you any harm. The reason you have it in some places and not in others is because in some instances you added the h1 when in Print medium, Flare added the autonum, but if you added the h1 when using default medium, it didn't.
By all means have a grumble about it (the x-ref thing annoys me daily) but don't fret that there's something broken - there isn't.
