I created a new topic and did the "Insert > Proxy > Insert TOC Proxy. The TOC Proxy dialog opens," but the dialog doesn't list any choices. It's empty.
I have nothing to choose, so I'm doing something wrong. Suggestions?

I don't get any choices either. I think you need to have defined a class for the TOC proxy ahead of time in order for there to be any options in the pulldown.
But it isn't necessary to make a selection there. If you click OK you should get a TOC proxy in the document, and you'll be good to go.
