Immediately after building an HTML Help target and opening it to view the results, I might get the message "The output for the Microsoft HTML Help target is not up-to-date. Do you want to regenerate it?"
What's up with that? I haven't changed a thing yet.

I would clean your build output directories in Flare, This can be done by Clicking on Build, and Clean Project

Make sure you're not building the html into your projects directory structure as well. (Building to the Output directory is fine).

I have having the same problem - every time I build my help project, then try to view it, I get a message saying the help isn't up to date and do I want to regenerate it? This is immeadiately after I built it.
I have already cleaned the project, and I am directing the build to the output folder. I really haven't changed anything in the project (except standard changes to the help topics) since Friday. This problem wasn't happening Friday, but it's happening every time I build today.
Does anyone have any other ideas why this is happening?

This has been bugging me for years. Thought I would come back here to see if I could find a solution. Once again, out of luck.
The two questioners above describe the problem well, as well as Flare's illogical behavior. I have always assumed this was a bug, as I can find no settings that seem related.
The two "solutions" supplied above make no sense whatsoever. In fact they are embarrassing.
How can my output be "out-of-date" when I just generated it? And the source files are still untouched? Obviously everything is up-to-date, is in synch.

I found a workaround of sorts.
Go to File > Options.
In the Build tab, turn on View output after successful build.
This way that useless message does not appear after the original build. However, if you close the output and click View, you still get the message (again, even if nothing is "out-of-date").
