I can't get toolbar captions to display in HTML Help output.
I have selected Display Button Captions under Button Options (HTML Help Setup tab). The preview in that tab looks like the first image below, but the second picture shows what I get after the build.
It looks like maybe the toolbar height is insufficient to display the captions, but I'm not sure how to change that and am looking for suggestions on that, or whatever else controls the display of captions.

I went into the skin editor and checked help buttons. The only other thing checked is to display button captions. What other settings are you using in your HTML Help Setup?

Try renaming or deleting the file hh.dat, which you should find in the hidden folder %UserProfile%\Application Data\Microsoft\HTML Help. So, typically:
C:\Documents and Settings\MichaelC\Application Data\Microsoft\HTML Help
It seems that if you restore the toolbar button text labels to a help window in which you previously removed them, the change is not evident until you get rid of the existing hh.dat file. Windows will automatically create a new version of this file when you next open any .chm file.
The function of the hh.dat file is to record the following about all the .chm files that you open:
