CHM Search Returns Multiple Instances of the Same Topic

Searches in my compiled HTML projects return two instances of several topic names. In each instance, clicking on one of the topic names results in the right pane displaying a message that the topic can't be displayed, while clicking on the other instance invokes the correct topic.

I've gone through the file list to check for duplicate topics but this doesn't seem to be the problem. Does anyone have any ideas why this might be happening or if there is another way to diagnose the problem?

its rare but it happens.

create a new TOC. and build from that.
try deleting the output folder, before you do anything,

in this folder C:\Program Files\MadCap Software\MadCap Flare V4\Flare.app\Resources\Bin

run itcc.bat

i know i am hitting this from all angles, but i remember doing some of these before, only thing is i can't remember which worked.
