PDF generation: Missing page numbers in index

To spare you time if you work with Flare version 4.2 and 5:
(addition to some information related the index proxy stated in other posts)

• clear the projects output folder
• clear the MadcapSoftware folder in c:/documents and settings/<username>/applications
• ensure, that the project and the output folder are located on the same local disk and there on the same partition, e.g. both on D:/

That's what we did to make the index proxy work properly again.
